The right forces for
extraction space closure.

With little crowding to relieve, closing four premolar extraction spaces becomes a demanding test of anchorage and force control. How do we move the anterior teeth while limiting molar anchorage loss and the bowing effect?

Dr. Takao Minamidate and Dr. Shuichiro Okano discuss a case treated with extraction of all four first premolars and two maxillary TADs. Inspired by a case presentation by a colleague, TADs were placed in the extraction sites and connected to the anterior segment with elastics to support torque and vertical control during space closure.

One case, two hours: the familiar Aligner Radio format. This time, we follow the case from space closure through finishing, comparing the ClinCheck plan with actual tooth movement and discussing the use of TADs and elastics. Join two clinicians for a detailed conversation about the course of treatment.

The discussion follows the sequence from canine retraction to incisor retraction and anterior space closure, then finishing with Class II and vertical elastics. Where should the anchorage come from, in which direction should force act, and how should the planned movement be staged?

The case used 48 initial aligners and one refinement of six aligners per arch. We will review the observed improvement in anterior protrusion and crowding, the Class I canine and molar relationships achieved, and the decisions that supported space closure.
